Service description
We carry out comprehensive assessments to inform the development of positive behaviour support guidelines, working together with the person, multi-disciplinary team, carers and other services. We also provide direct support if appropriate.
People who use our service can expect to receive an initial contact from a qualified practitioner to gather assessment information and gain consent. If it is decided our service will provide the most appropriate support/help, you will have a named nurse who will talk to you and/or your carers about how the team will work with you; whether we need to meet and talk to you, talk to other people or observe how you are. We would gather this information together and share it with you and your carers to help everyone consider if and how changes can be made to help support you.
The team currently consists of eight qualified nurses (RNLD) five healthcare support workers, an occupational therapist and a speech and language therapist, with support from psychiatry.
Referral criteria
Adults aged 18 years + with a diagnosed learning disability and their carers where the person with a learning disability has challenging behaviours that might mean they cannot continue to be supported in the community/at home.
