Adult Community Mental Health Teams

Adult Eating Disorders Service
<p>We offer an outpatient service for patients from Leicester, Leicestershire and Rutland and a regional inpatient service for the East Midlands.</p> <p>If you are worried about yourself or a loved one please refer to our <a href="http://www.leicestereatingdisorders.co.uk.">Frequently Asked Questions page on our website</a> for advice.</p> <p>Once a referral has been received by us, it will be triaged and if appropriate, you will be contacted to make an appointment for assessment. Following assessment you may be signposted to other services, offered individual or group therapy or inpatient treatment, depending on your needs. We work with you to achieve your goals in recovering from your eating disorder.</p> <p>Our service works in accordance with national guidance for the management of eating disorders set out by the National Institute for Health and Care Excellence (NICE).</p>

Children's Occupational Therapy Service

Department of Clinical Neuropsychology
<p>The Department of Clinical Neuropsychology is based at the Leicester General Hospital. We see both inpatients and outpatient who are having cognitive difficulties as a result of a neurological condition. These difficulties may be due to traumatic or acquired brain injuries, as well as chronic and progressive neurological conditions.</p> <p> </p> <p> </p>

Community Hospitals
<p>There are 13 wards situated in eight hospitals around Leicester, Leicestershire and Rutland, providing sub acute, complex rehabilitation and End of Life Care.</p> <p>Ward 1 at St Luke’s Hospital and Ward 2 at Coalville Community Hospital are both stroke units caring for patients following the acute phase of a stroke.</p> <p>Medical cover is provided by Advanced Nurse Practitioners (ANPs) with consultants visiting the wards twice weekly. Physiotherapy and occupational therapy is provided and all patients receive appropriate therapy assessments, including consideration of their psychological and cognitive needs, provision of goal-led intervention planned with the patient, together with holistic discharge planning. Care is planned, managed and overseen by registered staff with the support of unregistered assistants and technical instructors. Specialist occupational therapy and physiotherapy is provided at the two stroke units. Multidisciplinary care is provided in partnership with other professions and providers.</p> <p>Adults requiring rehabilitation, palliative and end of life care or sub-acute care will be accepted onto the wards following a handover from the referring ward/GP. Continence Service

<p><strong>Enhanced Respiratory Assessment Service (ERAS)</strong> is an admission avoidance service providing specialist care by the Community Integrated Respiratory Service (CIRS) for patients experiencing exacerbations of their COPD (ECOPD). In these cases patients have a sustained worsening of symptoms beyond their usual day-to-day variations and need specialist support to stay at home.</p>

<p>A mental health service for people in Leicester who are homeless or do not have permanent accommodation/tenancy (eg. hostel, temporarily staying with friends/family).</p>

Integrated Care Team
<p>Our Integrated Care Team provide specialist mental health assessment and treatment within primary care settings (eg GP surgeries and health centres) for adults aged over 18 years who have a long-term condition which is linked to mental ill-health.</p>

Integrated Community Specialist Palliative Care service
<p>The Integrated Community Specialist Palliative Care service is a team of nurses and HCAs from LPT and LOROS. The team look after patients with life-limiting illnesses, including cancer, who have complex palliative care needs, especially pain and symptom management, as well as patients who are in the last days of life. This can include when appropriate intervention has failed to control symptoms; when symptoms are escalating and there is incomplete symptom control; when patients or families/carers require psychological and spiritual care or if there are any other problems that other healthcare professionals are unable to manage.</p> <p>The team offers different levels of support to patients, families/carers, including; telephone advice, one-off assessments, on-going management and personal care.</p> <p>Working in partnership with other professionals, including community nurses, GPs, palliative care medical teams, and social services. They offer advice and support to other members of the community health service about their patients.</p>

Learning disabilities crisis response intensive support team
<p>Support for people with a learning disability who are over the age of 18, who are using behaviour that hurts themselves or other people. The staff/carers/social worker may be thinking that going into hospital or moving home may help. The Crisis Response Intensive Support Team (CRIST) will try to support people to stay in the community.</p> <p>The service offers initial risk assessment and risk management interventions and a 12-week Positive Behavioural Support Intervention model may be offered additionally.</p> <p>CRIST are open 7 days a week, including bank holidays from 8 am -9pm.</p>

Looked After Children's (LAC) Team
<p>The Looked After Children’s Health Team (LAC Team) aim to promote the health and well-being of children and young people who are looked after, including unaccompanied asylum seeking children.</p> <p>An Initial Health Assessment for all children and young people will be undertaken by a Paediatrician, following that, children and young people age 5-18 years will have a Looked After Children’s Nurse and children under 5 years will be cared for by their Public Health Nurse- Health Visitor.</p> <p>The team supports the child/young person and their carers throughout their time in care with health assessments, packages of care, advice, attending review meetings and making referrals to other services when required.</p>

Mental Health Services for Older People (MHSOP) Community Mental Health Teams
<p>Our MHSOP Community Mental Health Teams provide more routine (non-urgent/non intensive) multi-disciplinary assessment and intervention for patients of any age with a complex presentation of dementia or those over 65 with a complex functional illness. This intervention takes place in a community setting, for example, patient’s own home. The team comprises of Psychiatrists, Clinical Psychologists, Mental Health Nurses, Occupational Therapists and Health Care Support Workers.</p>

<p>The mental health urgent care hub is based at the Bradgate Unit, on the site of Glenfield Hospital, and is staffed by mental health practitioners with the expertise to treat people of all ages; this includes mental health nurses, support workers, and consultants. PAUSE/At Risk Mental State (ARMS)
<p>The PAUSE Team is LPTs At Risk Mental State provision. PAUSE stands for Psychological Awareness of Unusual and Sensory Experiences. The service provides psychologically informed assessment, formulation, and intervention to individuals aged 14-35 who are at high risk of developing psychosis. It is helpful to see PAUSE as a step before the PIER Team (Early Intervention in Psychosis service).</p> <p>Types of experiences that might be consistent with an At Risk Mental State include:-</p> <ul> <li>Seeming muddled, losing concentration, problems with memory, notable disorganisation.</li> <li>Thoughts feeling speeded up or slowed down</li> <li>Voice hearing, paranoia, unusual beliefs (but not so pronounced they require antipsychotic medication)</li> <li>Marked and often sudden decline in overall level of functioning</li> <li>Social withdrawal / discontinuation of employment or education</li> <li>Deteriorating relationships with friends and family</li> <li>Constant tiredness / sleep disturbance / loss of energy and motivation</li> <li>Appetite changes</li> <li>Feeling ‘uneasy’, irritable / Mood swings</li> <li>The above may be in the context of longer term anxiety or mood difficulties</li> </ul> <p>The ethos of PAUSE is to create non-labelling, non-pathologising narratives for understanding unusual and often extremely distressing experiences. The service offer is based upon psychological and social interventions.</p>

School Aged Immunisation Service
<p>The School Aged Immunisation Service supports and delivers the School Aged Immunisation Programme across Leicester, Leicestershire and Rutland. The service provides immunisations for children and young people who are in school or home educated. Vaccinations are provided either in schools or through community clinics and in some cases, through home visits.</p> <p>The service is provided and supported by a team of skilled and experienced staff. Our immunisation nurses have experience in all aspects of immunisation and vaccination.</p> <h3>The service offers the following immunisation programmes:</h3> <ul> <li>Seasonal flu vaccination to protect healthy children against flu (in school years Reception – year 11)</li> <li>Seasonal flu vaccination to protect children in special schools against flu</li> <li>Human Papillomavirus (HPV) vaccination to protect young people against various forms of cancer (offered from year 8)</li> <li>3-in-1 teenage booster and Meningitis ACWY vaccinations to protect young people against tetanus, diphtheria and polio and meningitis strains A, C, W and Y (offered from year 9)</li> <li>Measles, Mumps and Rubella (MMR) vaccination for under immunised young people (offered from year 8 and by invitation only)</li> </ul>

<p>The Agnes Unit is an inpatient service for adults (aged 18 years and over) with learning disabilities.
