Service description
An assessment will be performed during the first booked visit and further care negotiated. A named nurse will be allocated who will take the lead in the care. We will liaise with other professionals as required.
All supplies required to deliver care will be arranged at this time if not already organised. If the child / young person is to be discharged from a hospital setting it is expected that they will have provided adequate equipment. Once received, the referral will be reviewed by a registered children’s nurse who will contact the professional and family as required. It is for the professional who referred to contact the family and provide the next step advice if a referral is not accepted. Patients can be seen:
- within the home/residential homes/clinics
- colleges/Schools/nurseries
- social care settings
Appointments where possible are made to fit around the child / young person’s needs. All appointments are planned and dependent upon service availability.
Referral criteria
The service is offered to families registered with a GP within the Leicester, Leicestershire and Rutland region.
